4-variable score
—
Use only when differentiating subtle left anterior descending (LAD) coronary occlusion from normal-variant anterior ST elevation. This score does not exclude acute coronary occlusion and must not delay urgent clinical assessment.

Equations
Smith–Driver 4-variable formula
Score = (1.062 × STE60V3) + (0.052 × QTc) − (0.151 × QRSV2) − (0.268 × RAV4)Variables: STE60V3 = ST-segment elevation measured 60 ms after the J-point in lead V3 (mm); QTc = Bazett-corrected QT interval (ms); QRSV2 = total QRS amplitude in lead V2 (mm); RAV4 = R-wave amplitude in lead V4 (mm). A score >18.2 supports subtle LAD coronary occlusion rather than normal-variant anterior ST elevation in the population studied; a score exactly 18.2 remains in the ≤18.2 category.
