Vancomycin Level Estimation Calculator
This preserves the legacy one-compartment structure with a safety correction that prevents elimination fitting across an intervening dose. It assumes a fixed 1000 mg/h infusion rate and first-order elimination. It does not use renal function, Bayesian population pharmacokinetics or estimate area under the concentration–time curve (AUC). Confirm dosing decisions using an approved local therapeutic drug monitoring method.
Patient and model
The default half-life is used unless at least two eligible declining levels permit log-linear estimation. Eligible levels are outside every infusion and its 2-hour post-infusion exclusion window. Levels used together to estimate elimination must follow the same most recent dose, with no intervening dose.
